Seeking a full time Medical Care Coordinator.


Progressive, dynamic human services organization is seeking a full time Medical Care Coordinator to educate people living with HIV about the disease, facilitate access to case management, medical care, medication access, oral health care, psychosocial support and referrals for services. Directly connect HIV+ individuals who have fallen out of or not yet in medical care. Provide transportation to medical and other appointments and intervention services, including initial assessments and active engagement with and referrals for physical health, oral health, mental health and case management services. Effectively utilize bio-psychosocial models as it relates to HIV care; provide assistance in obtaining medications and enrollment in benefit programs; provide case management services including interviewing, assessment, service planning, follow-up and evaluation; provide education regarding safer sex practices, medication adherence and the importance of linkage to medical care. Serve as a liaison with medical providers. Work independently and as a member of a small team.

Requires a minimum of a bachelor’s degree in social work or related field, two years’ experience, LSW or LSW -eligible. Must demonstrate sensitivity to the LBGTQ and minority populations, especially those living with chronic disease. Must possess excellent skills in interviewing and engaging clients as well as in the provision of holistic assessment services. Ideal client has knowledge about HIV/AIDS service networks, entitlement programs and other community services. Requires excellent verbal and written communication skills and basic computer abilities. Must have transportation and be able to transport others
