Seeking a full time Non-medical Case Manager to provide services to individuals and families affected by HIV/AIDS, homelessness, mental health and substance abuse.  Interest in working with a diverse team devoted to the provision of care to a multicultural, diverse population essential.
• Provide case management intervention services, initial needs assessment, service plan, client teaching
• Facilitate access to medical care, medication, and referrals for a broad range of services.
• Provide assistance obtaining health insurance, benefits/entitlement programs, and other sources of support for covering costs of medications and medical care.
• Provide education regarding safer sex practices, medication adherence and the importance of linkage to medical care.
• Serve as a liaison between clients and medical/service providers.
• Provide transportation for clients
Requirements:  Minimum of bachelor’s degree in social work or related field, with two years of social service experience.  Knowledge about HIV/AIDS service networks, entitlement programs and other community services.  Must have reliable transportation, drivers license, insurance.

Seeking a Housing Retention Specialist to assist the housing team in their support of clients living with HIV in a site based PSH housing program.  Ensuring the safety of tenants and of property. This position is key in providing a safe and secure environment for tenants and supports the role of the Site Based Housing Manager

Interest and knowledge about Harm Reduction and Housing First models, working with people experiencing homelessness, and community resources.  Caracole is looking for an individual with proven ability to work with diverse populations, with the goal to maximize client’s self-sufficiency.

Qualifications include a high school diploma or equivalent and the ability to support adults in a site based housing model. This role requires basic knowledge about HIV/AIDS and the needs of people experiencing homelessness living with co-occurring, chronic illnesses with a particular focus on housing stability. This role requires effective communication and documentation skills as well as the ability to work autonomously. This role requires an ability to work within a larger team and be able to respond to crises in a professional manner.

Applicant must be detail oriented individual with good interpersonal communication skills having the ability to work independently and within a team. Ideal candidate should have knowledge and or experience with subsidized housing programs.

Please note that in order to ensure 24/7 coverage at the housing facility, work hours/days for this position may vary. Applicants should be prepared to work evening/night and/or weekend shifts, if necessary
