Caracole, Inc. is hiring for a Medical Care Coordinator to educate those living with HIV about the disease, facilitate access to case management, medical care, medication access, oral health care, psychosocial support and referrals for a broad range of all services. Primary duties include, but are not limited to, directly connect HIV+ individuals who have fallen out of or not yet in medical care, provide transportation to medical and other appointments, provide intervention services, including initial assessments and active engagement with and referrals for physical health, oral health, mental health and case management services, Effectively utilize bio-psychosocial models as it relates to HIV care, provide assistance in obtaining medications and enrollment in benefit programs, provide case management services including, interviewing, assessment, service planning, referrals, follow-up and evaluation, provide education regarding safer sex practices, medication adherence and the importance of linkage to medical care, Serve as a liaison with medical providers.

A minimum of a bachelor’s degree in social work or related field, LSW with two years’ experience. Demonstrates sensitivity to the LBGTQ and minority populations, especially those living with chronic disease. Must possess excellent skills in interviewing and engaging clients as well as in the provision of holistic assessment services, service planning, service delivery and program evaluation. Knowledge about HIV/AIDS service networks, entitlement programs and other community services. Proven ability to work successfully with staff, clients, their families and support systems, other disciplines and community agencies. Must possess excellent verbal and written communication skills with basic computer abilities. Must have transportation.
